When we cross a road there is a high chance we might die. This chance of dying increases when we are crossing a road in India! However, we still cross a road every single day!
Besides this innocuous activity, there are bad side effects of some daily activities that we routinely do without searching the internet for its inherent side effects-
1. Taking paracetamol (Tylenol) for fever. You can die of internal bleeding!

But when it comes to taking medicines that are FDA approved and have a long history of safety we must consult Dr Google.
Over 300 million people have received Finasteride till date. But only 39 have reported permanent erectile dysfunction after taking Finasteride. Is this percentage even statistically significant?

Bearing all this in mind and the drawbacks of a hair transplant without Finasteride, I would advise you especially if you are extensively bald and available grafts are limited, to stay away from a hair transplant procedure.
Clinics that hold commercial interests above the patients good interests in mind have meantime invented a middle path which is expedient in nature to assuage a patients feeling so that he signs up for the hair transplant. They convince the patient that if he does not want to take Finasteride, there is no problem at all for they have other ways to make the hair grow-
They are fully aware than one of these so called alternatives are a patch on the umbrella that Finasteride provides to the grafts to save them in the growing period from the affects of DHT.
It is mere commercial gimmickry and nothing else. Most of these clinics are part of the Hair Transplant Black Market.
The bottom line is that Finasteride is a must for growing hair that are susceptible to DHT. You must know that grafts from the so called permanent zone are only relatively permanent and they do carry on them DHT receptors, though to a lesser degree. This is the basis of use of Finasteride in hair transplant.
Since you do not get a hair transplant every other day (nor is it possible)- why not give this chance your best shot!
